Qualified Social Worker

A unique opportunity has arisen for a Qualified Social Worker to join Physical Disability and Sensory Impairment (PDSI) team in the South East London area.

Responsibilities:
1 Act as the single point of contact (a hub) for ABI referrals from hospitals, therapeutic centres and the community.

2 To chair and lead the ABI Virtual Team.

3 Co-ordinate initial holistic assessments, identifying the main care needs and setting up care plan prior to referring to the most appropriate agencies

4 Be actively involved in discharge planning from local and specialist units

5 Review and update current care pathways for individuals with ABI.

6 Act as a link for individuals and families of patients with an ABI to enable them to access appropriate support and services.

7 Review ongoing care needs to reduce crisis management, prevent institutionalisation and increase independence.

8 Ensure a structured/seamless handover between a variety of statutory and independent services when care needs change or new ones are identified.

9 Actively monitor and measure the effectiveness of the service.

10 Effect efficiency savings where ever possible, ensuring that services are commissioned appropriately and cost effectively.

Pay rate is between £24 and £29 per hour.

GSCC Registration is essential.
